UPMC Clinical Pharmacist Specialist I - Critical Care in Erie, Pennsylvania

To act as a consultant to physicians, nurses, pharmacists and other healthcare professionals in the areas of pharmacy, pharmacology and clinical pharmacokinetics and to facilitate cost effective patient care.

Responsibilities:

  • Identifies patients who are candidates to receive home intravenous therapy (i.e., nutrition, antibiotics, analgesics) and coordinates their discharge through Great Lakes Home Health Care. Supervises and trains Pharm.D students and pharmacy residents in rotation as assigned.

  • Represents the department on medical staff committees as assigned.

  • Participates in clinical pathway development.

  • Provides continuing education to pharmacists and other healthcare personnel as requested and obtains at least 40 acceptable CE credits/year and remains competent as a pharmacist as determined by the departmental competency statement and acts as a clinical pharmacist when requested.

  • Participates in medical student and resident education.

  • Monitors the drug therapy of selected patients, serving as an expert clinician, to promote the safest, most cost-effective and rational approach to their drug therapy management, and provides quality and timely drug information and consultation to physicians, pharmacists, nurses and patients.

  • Possesses the ability to obtain and interpret information considering age specific needs.

  • Provides cost-effective clinical pharmacy services including parenteral nutritional consults, clinical pharmacokinetics monitoring, target drug monitoring, CPR pharmacy services, drug information services, medication histories, discharge counseling and other patient care pharmacy consults when requested.

  • Coordinates the unique and routine pharmaceutical needs of selected patients to assure safe, cost-effective, rational drug therapy. Provides quality drug information to physicians, nurses, ancillary personnel and patients.

Qualifications:

  • Doctor of Pharmacy degree from an accredited college of pharmacy or Board Certified Specialist.

  • Have successfully completed a pharmacy and/or specialized residency accredited by the American Society of Hospital Pharmacists and/or fellowship, and have in-depth experience in drug therapy monitoring and patient care.

  • Four years experience preferred Licensure, Certifications, and Clearances: Pennsylvania employees only: Licensed to practice in the State of Pennsylvania. Maryland employees only: Pharmacist License from the State of Maryland required.
