Providence St. Joseph Health Senior Discharge Planner in Everett, Washington

Description Senior Discharge Planner - Providence Regional Medical Center Everett - Colby Campus $5,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment. SCHEDULE: 0.78 FTE (averaging 31.2 hours per week/62.4 per pay period). 9-hour shifts. Week 1: 3 working days. Week 2: 4 working days. 7am-4:30pm PST. Every other weekend rotation. Plans and coordinates activities related to transition planning. Develops and implements a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care. Responsible for the collaboration with medical staff, patient, family, community agencies, or facilities involved in patient's care, and the health care team by assessing, facilitating, planning and advocating for health needs on an individual basis resulting in quality, cost-effective outcomes. Required Qualifications: Master's Degree: Social Work or Counseling Upon hire: Washington Clinical Independent Social Worker License Or Upon hire: Washington Advanced Social Worker License Or Upon hire: Washington Mental Health Counselor License Experience in discharge planning, care coordination, or community resource navigation, in a clinical or community setting. Preferred Qualifications: Experience in discharge planning for medical and or surgical patients in an inpatient or outpatient setting. Experience in an acute hospital setting doing discharge planning is preferred. Why Join Providence?
